Office Policies
Appointments
We see patients by appointments only. Same day appointments are usually available for urgent or sudden illness for established patients. If you have any changes to your demographic information (insurance or address) please arrive 15 minutes prior to scheduled time.
Same Day Appointments
We have a limited number of same day appointments available for sudden illness. Please contact the office by 10:30 am if you wish to be seen same day.
Walk-in Appointments
We may occasionally be able to accommodate walk in appointments. Patients who walk in may be asked to wait or come back at a later time.
Cancellations/Rescheduling
All appointments require 24 hour cancellation notice. If we do not receive 24 hour notice prior to cancellation, you may be charged a $25 cancellation fee.
Late for Scheduled Appointments
We know your schedule is busy and that your time is valuable. We make every effort to keep on schedule. If you have been waiting more than 20 minutes and have not been advised of a delay, please speak to the receptionist to ensure that you have been properly checked in. Patients are seen in schedule order, with few exceptions. Patients arriving more than 10 minutes after their scheduled appointment may be asked to reschedule and may be charged a $25 cancellation fee.
Test Results
Our office will only contact you if your tests are abnormal. A staff member will contact you if you need to take immediate action regarding your results. If the nurse practitioner needs to speak to you regarding your results, you will be contacted and be asked to schedule an appointment. We do not print copies of lab results. They can be obtained on the patient portal.
Prescriptions and Refills
If you are on a medication that requires refills for a chronic disease (for example, high blood pressure or diabetes, depression), you will be given ample refills for 30 or 90 days at a time during your office visit.

Vaccines
We carry limited vaccines. At this time we do not carry vaccines for patients under the age of 7, excluding the influenza vaccine. Please contact the office for specific vaccine requests prior to scheduling appointment.
Forms Completion
Disability, Insurance Forms, Travel Forms, Release from Work, Prior Authorizations, and other forms are not required by all insurance plans or employers. If you require Christine to complete these forms, there will be a $25 charge in addition to your office visit charge.
